I had the same thing happen today - but fortunately spotted exactly where the ball went missing. It was during the trip to America multiball, and the ball was lost when it went down the centre scoop.
The mode continued with a single ball, until it eventually drained... then the table went into...
Damn... I thought I was going to get the scoop on this! ... I was poking around inside the new iOS app file earlier this morning, and noticed there's a configuration file enclosed for a table called "NewTaxi", with a Williams manufacturer tag :)
Though they did say that 4x anti-aliasing was now turned on for iPad 3... so although it's using the same textures, they'll be rendering a whole lot smoother.
