Some observations about shake nudge with the latest release on iPad 2:
Variable sensitivity via the angle of the device is most pronounced in landscape mode - still doesn't take much to tilt as the device approaches vertical.
In portrait mode, this is at least tempered, or may have a fixed sensitivity - don't have the same problems with tilting in this mode, even when vertical. In fact, with the iPad at about a 25 - 30 degree angle on the iCade, it works pretty danged close to how it should, except perhaps not sensitive enough. There may even be a bit of forward nudge.
Can't speak for iPhone - if you are playing on one of those, your eyes are a lot better than mine!
