It sounds like you accidentally pressed a wrong button. On an Xbox controller, pressing the "y" toggles this. On a keyboard, "H".

It may be time to start methodically hitting keys until you see the lock icon pop up. As for the black bars, that seems to happen on certain graphic cards only. On my Nvidia, I get no bars horizontally nor vertically.

There is a screen lock, but I use a controller, so I don't know what key activates it.

Ok, I just tested it. The iCade works just fine.

No kidding it's been a long time. My batteries are dead. I'll have to come back here later with my results. In the meantime, have you tried the controller with any other apps? If they don't work, check to make sure the controller is "paired" with the iPad properly.
