Table data has been updated.
Try going to Settings and Options > Data management > Manage tables, and delete Bone Busters.
Reopen the table to download the update.

Try checking the Game Cache.
Steam Library > Right Click Pinball Arcade > Properties > Local Files > Verify Integrity of Game Cache.

This is happening when you are playing a game with the Operator Mode On?
Is it at a specific point in the game?
Which adjustments did you change?
Did you try deleting Rescue 911 from Options > Manage Tables? And you now have v3?
The DS4 controller is not plug and play, you do need a driver. I actually prefer the DS3 (which might be a bit cheaper) that also needs a driver but seems to hold up better, mini usb vs micro usb.
TPA only works with the controller set to player 1.
Have you already tried mapping the controller in game via the Help & Options > Controls > Controller functions?
Does the controller show you as Player 1? Upper left light around the X on the 360 controller.
