On Pinball Arcade I just use AAA since no one else will really see them. On a real pinball or arcade machine I'd use BUM.

I have encountered this as well, thought I had been cheated of the goal the first time I completed the Gangway levels... but I figure it didn't unlock since you aren't actually lighting the Extra Ball in these instances. So I wouldn't call it a bug.
